Unexplained cough: pericardial cyst innocent bystander or culprit?

Abstract: Pericardial cysts are rare mediastinal cysts and an uncommon cause of cough. Patients are often asymptomatic but can present with cough, dyspnea and atypical chest pain. We describe the case of a 30 year old female with a 3 month history of unexplained cough. Extensive workup and empiric treatment for the standard causes of cough were unsuccessful. More detailed workup including bronchoscopy, endoscopy, pulmonary function testing and otolaryngology evaluation was normal. Computed tomographic (CT) scan and Magnetic resonance imaging (MRI) of the chest revealed a 5.8 cm pericardial cyst in the left cardiophrenic angle. Video assisted thorascopic surgery (VATS) was performed for removal of the cyst and the cough resolved completely postoperatively.

Abstract: Elizabeth C Hersch, Sharon FalzgrafVA Puget Sound Health Care System, Tacoma, Washington, USAAbstract: More than 50% of people with dementia experience behavioral and psychological symptoms of dementia (BPSD). BPSD are distressing for patients and their caregivers, and are often the reason for placement into residential care. The development of BPSD is associated with a more rapid rate of cognitive decline, greater impairment in activities of daily living, and diminished quality of life (QOL). Evaluation of BPSD includes a thorough diagnostic investigation, consideration of the etiology of the dementia, and the exclusion of other causes, such as drug-induced delirium, pain, or infection. Care of patients with BPSD involves psychosocial treatments for both the patient and family. BPSD may respond to those environmental and psychosocial interventions, however, drug therapy is often required for more severe presentations. There are multiple classes of drugs used for BPSD, including antipsychotics, anticonvulsants, antidepressants, anxiolytics, cholinesterase inhibitors and NMDA modulators, but the evidence base for pharmacological management is poor, there is no clear standard of care, and treatment is often based on local pharmacotherapy customs. Clinicians should discuss the potential risks and benefits of treatment with patients and their surrogate decision makers, and must ensure a balance between side effects and tolerability compared with clinical benefit and QOL.Keywords: dementia, management, behavioral symptoms, psychological symptoms
A Review of Summarizing and Main Idea Interventions for Struggling Readers in Grades 3 Through 12: 1978–2016

Abstract: This systematic review examines the effects of summarizing and main idea interventions on the reading comprehension outcomes of struggling readers in Grades 3 through 12. A comprehensive search identified 30 studies published in peer-reviewed journals between 1978 and 2016. Studies included struggling reader participants in Grades 3 through 12; targeted summarizing or main idea instruction; used an experimental, quasi-experimental, or single-case design; and included a reading comprehension outcome. A meta-analysis of 23 group design studies resulted in a statistically significant mean effect of 0.97. Group size, number of sessions, grade level, and publication year did not moderate treatment effect. Visual analysis of six single-case designs yielded strong evidence for retell measures and a range of evidence for short-answer comprehension measures. Findings suggest that main idea and summarizing instruction may improve struggling readers’ main idea identification and reading comprehension. Electronic Noses and Tongues: Applications for the Food and Pharmaceutical Industries

Abstract: The electronic nose (e-nose) is designed to crudely mimic the mammalian nose in that most contain sensors that non-selectively interact with odor molecules to produce some sort of signal that is then sent to a computer that uses multivariate statistics to determine patterns in the data. This pattern recognition is used to determine that one sample is similar or different from another based on headspace volatiles. There are different types of e-nose sensors including organic polymers, metal oxides, quartz crystal microbalance and even gas-chromatography (GC) or combined with mass spectroscopy (MS) can be used in a non-selective manner using chemical mass or patterns from a short GC column as an e-nose or “Z” nose. The electronic tongue reacts similarly to non-volatile compounds in a liquid. This review will concentrate on applications of e-nose and e-tongue technology for edible products and pharmaceutical uses.

Abstract: We shall recruit older people with dementia who have unplanned acute medical admissions and measure the prevalence of BPSD using the Behave-AD (Behaviour in Alzheimer's Disease) and the CMAI (Cohen Mansfield Agitation Inventory). Pain prevalence and severity will be assessed by the PAINAD (Pain Assessment in Advanced Dementia) and the FACES pain scale. We will then analyse how these impact on a variety of outcomes and test the hypothesis that poor management of pain is associated with worsening of BPSD.By demonstrating the costs of BPSD to individuals with dementia and the health service this study will provide important evidence to drive improvements in care. We can then develop effective training for acute hospital staff and alternative treatment strategies for BPSD in this setting.Dementia is common in older people admitted to acute hospitals in the United Kingdom (UK), affecting 42% of adults over 65 years with an unplanned medical admission. These patients have high mortality with a quarter of those with severe impairment dying during the index hospital admission [1]. Dementia significantly increases the length of hospital admission [2-5], complications [4] and the risk of iatrogenic harm through polypharmacy [6]. A number of recent documents including the English National Dementia Strategy, the National Dementia Research Summit and Alzheimer's Society "Counting the Cost" report have raised concerns regarding the quality of care received by people with dementia in acute hospitals and have highlighted lack of original research in this field [7-9].The term "behavioural and psychological symptoms of dementia" (BPSD) encompasses a range of symptoms including agitation, aggression, delusions, hallucinations, depression and apathy. Abstract: Se estudió la manipulación y preferencia floral de Centris rodophthalma Pérez en 3 localidades de la IV Región de Chile, caracterizadas, principalmente, por representantes de la familia Fabaceae y condiciones ambientales estrictas que reducen el rango de recursos disponibles para la abeja. Se estimó la influencia de factores florales de 6 especies de plantas en la preferencia de C. rodophthalma, con el objeto de determinar el grado de correspondencia entre la morfología de la abeja y la elección de la flor, considerando: longitud de la lengua de la abeja; tama o, dise o, color y estructura de la flor; profundidad de la corola; capacidad volumétrica de nectarios y medida del ángulo formado entre nectario y resto del estandarte. De estas variables, sólo las diferencias que muestra el ángulo formado entre nectario y resto del pétalo permiten discriminar entre las flores, dificultando la llegada de la lengua de la abeja al nectario, lo que podría determinar la elección floral de C. rodophthalma. Además se describe la manipulación de la flor por C. rodophthalma cuando va por polen o sólo por néctar. Abstract: We present the densest known packing of regular tetrahedra with density phi = 4000/4671 = 0.856347... Like the recently discovered packings of Kallus et al. [arXiv:0910.5226] and Torquato-Jiao [arXiv:0912.4210], our packing is crystalline with a unit cell of four tetrahedra forming two triangular dipyramids (dimer clusters). We show that our packing has maximal density within a three-parameter family of dimer packings. Numerical compressions starting from random configurations suggest that the packing may be optimal at least for small cells with up to 16 tetrahedra and periodic boundaries.
